What To Bring To Your Appointment

The Social Welfare Board houses a medical clinic, dental clinic, and women’s health/family planning clinic (called the Westside clinic). We also have a dispensary for medications and provide dietary counseling and glucose testing strips. Our income guidelines can be found here.

  • SOCIAL SECURITY CARD OR EQUIVALENT IDENTIFICATION

  • PHOTO ID OR BIRTH CERTIFICATE

  • PROOF OF RESIDENCE

  • PROOF OF PERSONAL AND HOUSEHOLD INCOME

  • CURRENT TAX INFORMATION MAY BE REQUIRED FOR MEDICATION ASSISTANCE

  • *Please note – Family Planning clients are exempt from these requirements.
